About Breast Augmentation

Cosmetic breast surgery is a procedure that redefines the breasts to create a fuller appearance. It is often considered to be one of the most wanted cosmetic surgeries, offering women who are unhappy with their breasts many options for improvement. Using implants, this procedure creates an appealing and natural-looking outcome by improving breast shape, size, and symmetry. Arlington, VA plastic surgeon Dr. Talal Munasifi is skilled in executing this surgery and performs over 4,000 breast augmentations each year. During your consultation at Advanced Plastic Surgery Center, Dr. Munasifi will listen to your concerns and desires and educate you on all of the options available, while also creating a surgical plan to meet your goals.

About Breast Implants

During your consultation with Dr. Munasifi, the correct implant will be recommended based on your body frame, as well as the results you wish to achieve. Implants come in different sizes and shapes — all of which are taken into consideration in order to attain the desired appearance. Today, the most common implants are saline or silicone, both widely chosen and safely approved by the FDA (Food and Drug Administration). It is important to mention that those considering saline implants must be 18 years of age, while patients must be at least 22 years old in order to receive augmentation with silicone. Aside from the type of augmentation chosen, there are other important factors to consider.

Saline vs. Silicone

Saline and silicone implants each have their own advantages and disadvantages. Both are safe, and one is not better than the other; it’s simply a matter of preference and what is best for your anatomy and aesthetic goals.

Silicone implants are filled prior to insertion and feel soft and natural. They are less prone to rippling, but ruptures can only be detected through an MRI. For this reason, women with silicone implants are advised to have an MRI every few years.

With saline implants, the shell is inserted and then filled with saline solution. This allows the size to be a bit more customized than silicone. It also means that the incision can be slightly smaller. They are more prone to rippling, but a rupture is immediately apparent as the implant deflates.

Size

Implants come in various sizes, from 110cc to 800cc. Dr. Munasifi will help you pick the perfect size to reach the shape and fullness you would like. During your consultation, we will take some time to have you try different implant sizes to find the best fit.

Shape

Breast implants are made in either a round or teardrop shape. Both shapes are commonly used; however, a large majority of our patients decide on a round-shaped implant. It often looks more natural and adds a fullness to the upper breast that a large percentage of patients desire.

Profile

Choosing the right profile is very important when planning your breast augmentation. We will help you navigate your options based on your body structure and aesthetic goals.

Cohesivity

Our team will help you determine the best level of cohesivity (or gumminess) for you.

Making Your Choices

These options may appear overwhelming, but agreeing to the correct implant is easy once the goals are discussed during the consultation. These considerations, combined with the expertise of Dr. Munasifi, ensure the right outcome is attained.

Surgical Technique

Dr. Munasifi will customize your breast augmentation for your anatomy and goals. There are a variety of options for incision locations. The incision can be made in the fold under the breast (infra-mammary), around the nipple (peri-areolar), or under the arm (trans-axillary, with endoscopic assist). In all cases, the incision is kept as short and inconspicuous as possible.

The implant can be inserted either under the breast tissue (“sub-glandular”), or partially under the pectoral muscle (“sub-pectoral” or “sub-muscular”). When the implant is placed in a sub-muscular position (under the breast tissue and the pectoral muscle), there is a lower chance of capsular contracture (shrinkage of the tissue capsule surrounding the implant, causing the implant and breast to feel unnaturally firm), and mammography may be more reliable. There is also less risk of visible or palpable implant edges through the skin.

Once Dr. Munasifi creates a “pocket” for the breast implant, a temporary “sizer” implant is placed in order to determine the proper shape and volume (size) of the final implant. Then, the final implant is chosen, inserted, and properly positioned within the pocket.

Dr. Munasifi uses the Keller Funnel “No Touch” technique when placing breast implants. This advanced surgical technique minimizes the potential for bacteria to come into contact with the implant during the augmentation procedure, resulting in greatly reduced chances of complications after surgery. After the implant is placed, he carefully checks to ensure that your breasts have the shape and size you anticipated pre-operatively.

What to Expect

After surgery, the incisions are closed with dissolvable sutures and surgical tape. This means no suture removal is required. All patients will be taken into recovery and observed and monitored before being released. It’s normal to experience some soreness, bruising, tightness, numbness, itching, and swelling. Prescription pain medication can be prescribed if necessary. Once Dr. Munasifi gives his approval, you will be sent home wearing a compression garment to assist with swelling. Within a few days, the swelling and bruising will start to decrease, and the breasts will begin to feel normal as they settle into their new position. Within your one-week post-operation visit, your gauze dressings will be removed.

The full recovery period is specific to each individual and the level of activity required for her daily functions. A large percentage of patients can return to normal activities within the first week; however, vigorous exercise should be avoided for a minimum of 6 – 8 weeks. You will also need to sleep on your back or side for a few weeks as you heal. Cosmetic breast surgery creates beautiful outcomes that look and feel natural. This treatment in most cases can be life changing for many patients, allowing them the chance to feel more self-assured.
